چکیده

A 54-year-old woman presented with a 3-month-history of blurred vision and refractory headache. Examination revealed severe bilateral visual acuity compromise, normal campimetry and bilateral papilledema. As a mean to investigate raised intracranial pressure, neuroimaging studies were performed and showed extensive sellar mass (Figure). Patient underwent transsphenoidal approach for resection of pituitary mass which disclosed metastasis of neuroendocrine lung tumor. Chest CT showed a mass in the anterior segment of right lower lobe of lung. Although symptomatic pituitary gland tumors are common in clinical practice, metastatic disease represent a rare cause of sellar tumors, mainly of breast and lung primary origin.

Sellar reconstruction without intrasellar packing after endoscopic surgery of pituitary macroadenomas is better than its reputation

OBJECTIVES Sellar reconstruction with intrasellar packing following endoscopic resection of pituitary macroadenomas remains a subject of clinical and radiological discussion particularly, when an intraoperative cerebrospinal fluid (CSF) leakage is absent. This study was conducted to contribute our experience with sellar reconstruction after a standard endoscopic surgery of pituitary macroadenom...

Postoperative sellar hematoma after pituitary surgery: clinical and biochemical characteristics.

BACKGROUND Postoperative sellar hematoma can develop following surgery for pituitary tumors and other sellar masses such a Rathke's cleft cyst (RCC) due to continued blood oozing. Though often mild sellar hematoma can create mass effects that might impair pituitary function. AIM This study summarizes the clinical and biochemical characteristics of sellar hematoma and explores the potential me...

DIAGNOSIS Lymphocytic hypophysitis should be considered in the differential diagnosis of pituitary masses and/or hypopituitarism in females who are pregnant or i...

Pituitary infiltration by non-Hodgkin's lymphoma: a case report

INTRODUCTION Pituitary adenomas represent the most frequently observed type of sellar masses; however, the presence of a rapidly growing sellar tumor, diabetes insipidus, ophthalmoplegia and headaches in an older patient strongly suggests metastasis to the pituitary. Since the anterior pituitary has a great reserve capacity, metastasis to the pituitary and pituitary involvement in lymphoma are ...

Management of Pituitary Adenomas: Mononostril Endoscopic Transsphenoidal Surgery

Introduction: The endoscopic transsphenoidal approach for pituitary adenomas and other sellar lesions is quickly becoming the procedure of choice in their surgical management. The most common approach is binostril three-hand technique which requires a large exposure and subjects both nasal cavities to potential trauma. To reduce nasal morbidity, we employ a mononostril two-hand technique with t...
